• Answer the following questions in their order
• After you complete the information, hit "enter" and then,
• click on the "submit" button that will appear just below the message box on the right

1.   What is your name as it appears on your drivers license?

2.   What is your physical or "911" address?

3.   What is your telephone number?

4.   Do you have auto insurance in effect right now?   If your answer is "yes", have you had insurance constantly with no lapse for at least 6 months and if so, what is your current company (not "agency") ?

5.   List every person in your household who has a driver's license and does not have their own automobile insurance policy.  List your and each person's date of birth; drivers license number and state of issue; social security number; any accidents or violations/tickets with the dates of each occurance.

6.   List every vehicle that you own  by showing the year model; the make (such as Ford or Toyota); the VIN# (the serial number); if the vehicle is used to drive to and from work or school (if yes, how far one way); whether you want liability only or full coverage.
